Care coordination incorporates the planning and delivery of care services to an individual in a logical, structured and time balanced approach. Our comprehensive approach focuses on identifying specific care needs and goals into a plan and progressively working toward goals.
The primary role of the care coordinator is to simply talk to the service user and assess their needs accurately, understand what is important in their life, and develop a plan that supports achievement of personal goals.
